New System to Improve Patient Categorisation: Officials
Srinagar, Apr 26: The Sher-i-Kashmir Institute of Medical Sciences (SKIMS) Soura has reorganised its Emergency Department into Red, Orange and Green zones, streamlining triage, staffing patterns and capping patient stay at 72 hours.
The move has come in view of the huge rush of patients in the emergency and ensuring proper patient categorisation, optimising staffing, reducing overcrowding, and enabling timely treatment and transfers based on patients’ clinical needs.
Officials said the new system aims to ensure proper patient categorisation and deliver care based on clinical requirements. The move follows an Emergency Medical Board meeting held earlier this year in February.
As per an official communication, six DM or Post-DM Senior Residents will be posted in the Medical Triage area on a rotational basis, with two present at any time. Triage services will function daily from 8 am to midnight, including Sundays, in two shifts.
The reception area will be managed by Non-DM Senior Residents from the Department of Emergency Medicine in three shifts, with two SRs per shift. They will act as the first point of contact for evaluation and initial management of emergency patients.
The communication mentions that the Red Zone (Outer Reception – Critical Care Area), comprising 12 beds for critically ill patients, will be managed by eight postgraduate students from the Department of General Medicine (second- and third-year PGs).
“Two PGs will be deployed per shift in a 24-hour cycle, ensuring continuous monitoring and stabilisation of patients before their transfer to specialty units. The stay in the Red Zone will be limited to 1–2 hours.” it said.
The Orange Zone (Inner Reception – Intermediate Care Area) will function round-the-clock under Non-DM Senior Residents on a shift basis, supported by Junior Residents for sampling and monitoring vital parameters. At any given time, two SRs and four JRs will be on duty. Patient stay in this zone will be under 24 hours.
The Green Zone (Medical Observation Ward), functioning like a General Medicine ward, will be managed by first-year postgraduate students along with Senior Residents as per routine protocols. One SR and one PG will be on duty at any time, with patient stay limited to 72 hours.
The communication further states that no patient will be allowed to remain in the Emergency Department beyond 72 hours, and any deviation will be reported to Director SKIMS and EOSG.
“The Control Room shall submit daily census reports to the concerned HODs and the Director SKIMS, indicating patient details, duration of stay and the consultant under whom the patient has been admitted,” it added.
Earlier this year, the valley’s major tertiary care institute modernised its Emergency Operation Theatre (OT) with the installation of state-of-the-art surgical equipment and advanced intervention facilities.
